How do I make a request for cheque book of 5 leaves?
Not possible because bank only issue minimum 10/Leaf Cheque
The above Jupiter fee link you posted itself mentions that chequebook would be of 5 leaves only.

As per my knowledge bank only give Minimum 10/Leaf of cheque maximum bank follow this.
May be Jupiter provides 5/Leafs only.

I know I am replying very late but this answer is for the people with same query still.
I ordered chequebook. They issue 5 leaves only. I ordered it from Help Section in Jupiter app by typing chequebook and follow the chatbot links.
It delivered by speedpost by India Post and not any consignment Number shared. You can try to get consignment no° by customer support but they cannot share I guess. You have to wait for chequebook. But most of time postman redirect it to bank again by excusing that incomplete address or other so keep an eye on it. Try to get in touch with your areas post office.
If post is comming via bluedart nowdays (as debit card) then this excuse will be definite so keep a contact with bluedart support.
